Nursing Home Care
For eligible Veterans, nursing home care may be provided within VA nursing home settings called Community Living Centers (CLCs), or through community nursing homes with which we contract via our Community Nursing Home Program (CNH Program). Our community living centers are located at Palo Alto, Menlo Park, and Livermore divisions. We may also assist in referring patients to community nursing homes.
A variety of services are offered, from short-term, post-hospital care to long-term care for Veterans who are not able to live independently outside a nursing home setting.
Palo Alto Division
The programs at Palo Alto division are primarily for hospice and palliative care and short-stay, sub-acute care. Sub-acute care is intended to provide a “bridge” between acute care hospitals and care elsewhere, either the home or a more typical nursing home setting.
Menlo Park Division
The Menlo Park community living center provides long-term care, nursing home level short-stay post-hospital sub-acute and rehabilitative care, hospice and palliative care, respite care, and specialized dementia and geropsychiatric care units.
The Livermore division community living center provides long-term care, nursing home level short-stay post-hospital sub-acute and rehabilitative care, and hospice and palliative care.
Sometimes nursing home care is needed for just a short time to help patients recover from a recent illness or to undergo short-term rehabilitation before going home. Some Veterans may require long-term nursing home care. Hospice, palliative, and respite care may also be provided in nursing homes. Because individual Veterans have very different needs and because availability of services may vary by area, we encourage to discuss your concerns about nursing home options with your primary care provider or a social worker.
