Construction in San Jose
VA Palo Alto Health Care System (VAPAHCS) began operations in San Jose in 1996 as an outpatient Community-Based Outpatient Clinic (CBOC), offering primary, specialty, mental health, and ancillary and diagnostic services. The original clinic operated within a 65,000 square feet, single-story facility located in south San Jose. VAPAHCS has received approval to enter into a new 20-year lease to replace the existing clinic. The replacement facility will be sized commensurate to the original clinic.
The new CBOC will feature VA's new Patient Aligned Care Team (PACT) design to support a Veteran-centric model of care. PACT's team-based care design also promotes staff collaboration in a flexible and adaptable concept.
The PACT design also features state-of-the-art training and patient education spaces will promote group classes. All clinical areas will be wired to support expansive Telehealth programs.
The new San Jose clinic will consist of administrative support spaces and medical clinic spaces distributed over three floors, including the following:
- Primary Care
- Mental Health
- Audiology and Speech Pathology
